Health insurance Illinois provides a list of eligibility criteria and premium cap. Federal eligibility applies in Illinois. There are three types of plans that are offered in Health insurance Illinois. First is Plan 2, which is only available to individuals who are enrolled in Parts A and B of Medicare due to an end-stage renal disease or a disability. Second is Plan 3, which is a PPO plan that is available to individuals who are not federally eligible and are consequently subject up to a six-month waiting period for a pre-existing condition. The third and last is Plan 5, which is also a PPO plan for individuals who are federally eligible. But in this plan, there is no pre-existing condition limitation and there exists a limit of 45 days per calendar year applicable in all hospitals for the treatment of mental illness of inpatients.

For those individuals who are not federally eligible and are applying for Plans 2 or 3 in a Health insurance Illinois must:
1. Be a permanent resident alien or a U.S. citizen;
2. Be a resident of Illinois for at least 80 days;
3. Have applied to an insurance company within the last 9 months and obtained a dismissal or denial to issue the insurance for health reasons by one insurer; or
4. Have obtained a dismissal or denial or renew considerably related individual health coverage at a charge surpassing the amount charged for the Health insurance Illinois plan; or
5. Have been diagnosed with one of 31 presumptive medical conditions.
